*In order to qualify for the sign-on bonus the stakeholder must be hired as a full-time R.N./LPN. The first payment of $500.00 will be made after the Stakeholder completes 30 days of employment with satisfactory or above performance. The second payment of $500.00 will be made after the Stakeholder completes 90 days of satisfactory or above performance. The third payment of $500.00 will be made after the Stakeholder completes a year of employment with satisfactory or above performance. The sign-on bonus is not part of base pay but is considered taxable income. The Stakeholder understands that the bonus is forfeited if the Stakeholder’s employment should terminate whether termination is voluntary or involuntary. Stakeholder must maintain a full-time status for a period of one year. Payments are forfeited if the stakeholder reaches the second level of attendance coaching and counseling; the fifth absence within a Stakeholder’s first anniversary year – written, or if their performance is not satisfactory.
